How Can I Prepare My Assessment?

How Can I Prepare My Assessment?

For your evaluation, you may want to comment on some of the things you need from the evaluation and the service and support planning process. You can then include these views in your assessment to help you discuss your needs.

If you want care worker jobs then you can contact 3 Trees Care & Support.

Here are some things to keep in mind before assessment:

  • What your needs are and how do you think social work and support can help you. For example, you may have personal care needs and you may need support in keeping your apartment clean and tidy and preparing meals. You can write comments on projects that need help.
  • The results you want to achieve. What are your wishes, hopes, and life plans? For example, you may want to stay in touch with family or friends, live in your apartment, find a job, or manage your finances.
  • What are the barriers to your success and life plans?
  • Do you want someone to support you in the assessment process, such as a family member, friend, or independent lawyer?
  • If your needs change. For example, you may feel good and lack care and support when preparing for the assessment. But it might be helpful to think about how your condition has affected you in the past. This will help you plan how it might affect you in the future.

What if my needs change from day to day or month to month?

Your situation may mean that you have fun when you don’t need much attention and support, and during bad times when your need is greatest. Your review should keep in mind that your needs may change (change in unexpected ways).

This means that we can:

  • Check on you for a long time, so that they are well aware of what your needs are and how it affects your life.
  • Do the research in no time, unless they make sure they have a proper description of all your needs.

How do I participate in my assessment?

It is extremely important that municipal areas participate as much as possible in your assessment. This means they have to do things like:

  • Contact an interpreter if you have difficulty communicating.
  • Involve your family and / or guardians if this helps you.
  • Support if you have difficulty understanding, preserving (remembering) or thinking (thinking about) information or have difficulty expressing your wishes and points of view.
  • Help yourself understand. They should explain the purpose of the assessment and how it was performed. They should also provide you with information if you ask and answer any questions you may have.

Can I reject an assessment?

Yes, if you reject your needs assessment, there is no obligation on municipalities to assess you. But there are exceptions.

You can not reject an assessment of need:

  • You do not have the ability to refuse, and we believe it is in your best interest to conduct an assessment
  • We believe that you have been harassed or neglected, or that you are at risk.

If you have rejected your opinion, but you have changed your mind and want to assess you, we will do so. Similarly, if you have rejected your assessment, but we believe that the situation has changed, we should make an assessment if it is your choice.
